We are seeking a dedicated individual to fill the role of Team Lead within our Carlisle & District Integrated Care Communities (ICCs), while also collaborating with specialist services across the Community Collaborative.

As a Team Lead, you will be instrumental in connecting ICCs with Specialist services, ensuring that we meet the operational needs of our community collaborative effectively. This role is designed for a proactive individual who thrives in dynamic settings and excels in guiding teams through various projects and tailored initiatives to foster innovation and teamwork.

Your leadership will be crucial in coordinating and enhancing the delivery of healthcare services, guaranteeing that patients receive personalized care of the highest standard. Your expertise will empower team members, cultivating a collaborative and nurturing environment that promotes professional growth and development.

Main Responsibilities

  • Support other Leads and Service Managers in executing tailored projects and enhancing team performance.
  • Become an integral part of the senior leadership team for the ICCs, aiding in the management and guidance of community nurses and therapists.
  • Assume delegated responsibilities for key service delivery areas, focusing on operational and performance management to meet constitutional standard waiting times and achieve financial stability.
  • Actively contribute to the accomplishment of objectives aligned with both corporate and collaborative agendas.
  • Participate in a shift system associated with this role outside of standard working hours to meet the demands of the base service and the broader Trust.
  • Be adaptable to changes in portfolios as required by the Trust to ensure optimal service delivery.
  • Work across multiple locations as necessary.
