We are seeking a highly motivated and experienced Community Staff Nurse to join our South West Neighbourhood Team at Shropshire Community Health NHS Trust.
Main Responsibilities- Deliver high-quality nursing care to patients in their own homes or chosen place of residence, promoting timely discharge and enabling people to realize their potential.
- Work autonomously as a practitioner, responsible for the holistic assessment of complex and simple care needs, and the development, implementation, and evaluation of care plans.
- Collaborate with other members of the multidisciplinary team to promote independence for patients on the team caseload.
- Meet the travel requirements of the post, working regular weekends and some Bank Holidays.
Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford, and Wrekin, and some services in surrounding areas. We are committed to delivering excellent patient care and modern ways of working.
Person Specification- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post-registration general experience
- Experience of working in a multi-disciplinary/multi-agency health/social care environment
- Experience and knowledge of basic administration systems, including Microsoft Office
- Experience of successful partnership working arrangements with other services
- Previous community experience
