About the Role

We are seeking a highly motivated and experienced Community Staff Nurse to join our South West Neighbourhood Team at Shropshire Community Health NHS Trust.

Main Responsibilities
  • Deliver high-quality nursing care to patients in their own homes or chosen place of residence, promoting timely discharge and enabling people to realize their potential.
  • Work autonomously as a practitioner, responsible for the holistic assessment of complex and simple care needs, and the development, implementation, and evaluation of care plans.
  • Collaborate with other members of the multidisciplinary team to promote independence for patients on the team caseload.
  • Meet the travel requirements of the post, working regular weekends and some Bank Holidays.

Person Specification
  • RGN NMC Registered
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
  • Post-registration general experience
  • Experience of working in a multi-disciplinary/multi-agency health/social care environment
  • Experience and knowledge of basic administration systems, including Microsoft Office
  • Experience of successful partnership working arrangements with other services
  • Previous community experience
