About the Role

We are seeking a highly skilled and compassionate Therapy Rehabilitation Specialist to join our multidisciplinary team at HCRG Care Group. As a key member of our Reablement Service, you will play a vital role in supporting individuals with physical and psychological needs during their rehabilitation journey.

Key Responsibilities
  • Work collaboratively with clinicians to develop and implement personalized assessment and intervention plans.
  • Provide rehabilitation and support services to patients in their own homes, promoting independence in daily living tasks and encouraging rehabilitation approaches to restore independence.
  • Carry out basic nursing care tasks as necessary, including continence management and prompting of medication.
  • Communicate effectively with colleagues, other agencies, and professionals to provide a coordinated service, including GPs, district nurses, social workers, and family and carers.
