About the Role

We are seeking a skilled Reablement Therapy Worker to join our team at HCRG Care Group.

The successful candidate will work with individuals with a range of physical and psychological conditions, providing rehabilitation and support to restore independence and promote functional activities.

Main Responsibilities

  • Support clinicians with assessment and intervention plans
  • Provide rehabilitation and support, including delivering equipment items and promoting independence in daily living tasks
  • Encourage rehabilitation approaches to restore independence
  • Carry out basic nursing care tasks as necessary, including continence management and prompting medication
  • Feedback progress of service users, liaising with colleagues and other agencies and professionals
