Job Title: Homelessness Inclusion Health Nurse

We are seeking an experienced and compassionate Homelessness Inclusion Health Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will play a vital role in providing high-quality, person-centered care to individuals experiencing homelessness.

Key Responsibilities:
  • Develop therapeutic alliances with individuals experiencing homelessness, using trauma-informed care principles to build trust and rapport.
  • Conduct holistic physical and psychosocial health assessments on the street, identifying unmet health needs and developing individualized care plans.
  • Provide immediate clinical and therapeutic interventions, including wound care, nurse prescriptions, and Patient Group Directions.
  • Work closely with healthcare agencies, including GPs, to ensure seamless care coordination and communication.
  • Advocate for individuals' rights and choices, promoting independence and self-care while identifying potential safeguarding concerns.
  • Document all clinical activity thoroughly, ensuring accurate and timely reporting of health needs and project outcomes.
Requirements:
  • Registered Nurse with experience in homeless health or a related field.
  • Strong understanding of trauma-informed care principles and their application in practice.
  • Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the needs of the service.
  • Knowledge of local healthcare services and systems, with the ability to navigate complex care pathways.
