Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, addressing their physical and mental health needs.

About the Role:
  • Provide healthcare and clinical interventions to homeless individuals, including assessments, treatment plans, and follow-up care.
  • Work collaboratively with outreach teams to identify and engage with individuals in need of healthcare services.
  • Develop and maintain relationships with local healthcare providers, social services, and other stakeholders to ensure seamless care coordination.
  • Conduct health education and promotion activities to empower individuals to take control of their health and well-being.
  • Participate in quality improvement initiatives to enhance the effectiveness and efficiency of our outreach services.

Requirements:
  • Registered Nurse with current registration and a valid NMC PIN.

  • Ability to work independently and as part of a team, with a flexible approach to working hours.
  • Knowledge of local healthcare services, social services, and community resources.
