About the Role

We are seeking a highly skilled and compassionate Nurse Specialist to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to our patients.

Key Responsibilities
  • Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the patient and their family.
  • Provide expert nursing care and support to patients with complex mental health needs.
  • Work closely with the multidisciplinary team to ensure seamless care coordination and effective risk management.
  • Develop and maintain strong relationships with patients, families, and carers to promote empowerment and independence.
  • Contribute to the development and implementation of evidence-based practice and quality improvement initiatives.
