Job Summary
We are seeking a compassionate and dedicated Community Support Specialist to join our team at Richford Gate Medical Centre. As part of our Primary Care Network (PCN), you will play a vital role in connecting patients with essential non-medical resources that can significantly improve their well-being and overall health.
Key Responsibilities
- Act as a bridge between GP surgeries and local community support services, identifying and addressing the diverse needs of our patients.
- Provide guidance and support to patients facing challenges such as housing issues, financial difficulties, social isolation, or access to essential care services.
- Develop strong relationships with local organizations and agencies to ensure a comprehensive network of support is available for our patients.
- Effectively communicate patient needs to the clinical team, facilitating a holistic approach to healthcare delivery.
Qualifications & Experience
