We are looking for a dedicated Healthcare Liaison Specialist to deliver a hospital-based service in Kent. In this role, you will collaborate closely with Accident & Emergency teams to create and execute care plans for individuals who frequently visit the hospital.
Utilizing the Making Every Contact Count (MECC) methodology, you will provide guidance, information, harm reduction strategies, and brief interventions.
Location: Kent
Working Hours: Full time, 37.5 hours per week
Salary: £27,449 - £31,529

Key Responsibilities
The Role:
As an integral member of our Reach Team in West Kent, your responsibilities will include:
Collaborating closely with Accident & Emergency teams to design and implement care plans for frequent hospital visitors. Conducting specialized assessments of patients admitted to the hospital for alcohol-related concerns using AUDIT screening tools, and providing brief interventions and referrals. Offering advice, support, and training to hospital personnel regarding drug and alcohol awareness, screening, assessment processes, and referral pathways. Creating and facilitating educational opportunities to enhance the knowledge and skills of clinical staff working with this demographic. Coordinating with community substance misuse teams to ensure effective follow-up for repeat presentations and hospital discharges. Keeping General Practitioners informed of patients' statuses and discharge plans/referrals within five days of their hospital discharge. Establishing specific connections with designated specialist acute services.About You:
