We are looking for a committed Healthcare Liaison Specialist to deliver a hospital-centered service in Kent. You will collaborate closely with Accident & Emergency teams to create and execute care plans for individuals who frequently visit the hospital.
Utilizing the Making Every Contact Count (MECC) framework, you will provide guidance and information, promote harm reduction, and conduct brief interventions.
Location: Kent
Work Hours: Full time, 37.5 hours per week

Key Responsibilities
Role Summary:
As an integral member of our Reach Team in West Kent, your responsibilities will include:
Collaborating with Accident & Emergency teams to design and implement care plans for frequent hospital visitors. Conducting specialized assessments of patients admitted to the hospital for alcohol-related concerns using AUDIT screening tools, and providing brief interventions and referrals. Offering guidance, support, and training to hospital personnel regarding substance use awareness, assessment processes, and referral pathways. Creating and leading educational opportunities to enhance the knowledge and skills of clinical staff working with this demographic. Coordinating closely with community substance misuse teams to ensure proactive and effective follow-up for repeat admissions and hospital discharges. Keeping general practitioners updated on patient status and discharge plans/referrals within five days of discharge. Establishing specific connections with designated specialist acute services.Candidate Profile:
