Position Overview

We are seeking a qualified Adult Social Worker to be part of a dynamic Hospital Discharge Team.

Role Requirements

This position necessitates a Social Work Qualification along with a minimum of two years of post-qualification experience.

About the Team

This team operates in a high-energy environment, dedicated to ensuring that patients are discharged from the hospital safely and receive the necessary support. Key responsibilities include conducting bedside evaluations, coordinating suitable care packages for thorough discharges, and advocating for the autonomy of service users. Facilitating access to essential resources and external support at the earliest opportunity is crucial for the success of this role. The team is present in the hospital five days a week, which is a requirement for this position.

Candidate Profile

The ideal candidate will have experience working within a hospital team or in a short-term assessment environment. A degree in Social Work (Degree/DipSW/CQSW) along with at least two years of experience in an Adult Social Work context is essential. A valid UK driving licence and access to a vehicle are also required for this role.
