Position Overview

An exceptional opportunity has emerged for a dedicated band 7 Nurse to join our esteemed team of advanced care planning coordinators within Cwm Taf Morgannwg University Health Board.

Within the palliative care service group, the successful candidate will play a crucial role in the organization, facilitation, and implementation of Advance Care Planning (ACP) for individuals, regardless of their mental capacity, to aid clinical decision-making at the end of life.

The individual will be instrumental in the ongoing enhancement of processes that support and integrate Advance Care Planning across primary care, the independent sector, and broader services.

The ideal candidate will exhibit outstanding communication and leadership abilities, demonstrating the capacity to work independently as well as collaboratively within a team. They will serve as a specialized resource for other health and social care professionals regarding matters related to advance care planning.

Key Responsibilities

• Advocate for the advantages and adoption of Advance Care Planning (ACP) with patients and their families in collaboration with the Primary Care Team.

• Collaborate as part of a multi-disciplinary team, promoting effective communication with colleagues across various disciplines, ensuring that all interactions are conducted with dignity and respect.

• Engage with Care Home Managers, District Nurses, General Practitioners, and Out of Hours medical services to provide information and guidance on Advance Care Planning (ACP).

• Assist teams and key health professionals in implementing the principles of ACP throughout Cwm Taf UHB in partnership with the Primary Care Team.

• As a team member, the candidate will be expected to operate across all localities based on service requirements. A valid driver's license and access to a vehicle are essential for this position.
