Job Summary:

Cheshire West and Chester Council is seeking a dedicated and compassionate Reablement Coordinator to join our team. As a Reablement Coordinator, you will play a vital role in supporting individuals in their own homes to regain their independence and confidence.

Key Responsibilities:

  • Deliver a high-quality reablement service to promote and sustain people's independence living in the community.
  • Work collaboratively with the reablement team to coordinate and complete service users' journeys, ensuring compliance with organisational policies and Care Quality Commission regulated guidance.
  • Develop and implement individualized plans to support service users in rebuilding their daily living skills and achieving their goals.
  • Conduct risk assessments and reviews to ensure the safety and well-being of service users.
  • Utilize developed IT skills to maintain accurate records and reports.

Requirements:

  • Experience in delivering frontline care, including reablement.
  • Knowledge of internal policies and procedures relating to social care, including Care Quality Commission standards and regulations.
  • Understanding of related legislation and manual handling practices.
  • Developed IT skills, including Microsoft Office.
