About the Role

Orri is seeking a highly skilled and compassionate Case Manager to join our team. As a key member of our care team, you will play a vital role in ensuring our clients receive exceptional support and care throughout their treatment journey.

Key Responsibilities
  • Client Onboarding: Welcome new clients and their families, ensuring a smooth transition into our care.
  • Treatment Planning: Collaborate with our clinical team to develop and implement personalized treatment plans, ensuring clients receive the best possible care.
  • Client Reviews: Conduct regular reviews with clients to discuss their progress, address concerns, and make adjustments to their treatment plans as needed.
  • Communication: Serve as a single point of contact for clients, their families, and external stakeholders, providing clear and compassionate communication throughout the treatment process.
  • Documentation: Maintain accurate and up-to-date records of client interactions, treatment plans, and progress.
  • Team Collaboration: Work closely with our multidisciplinary team to ensure seamless care coordination and effective communication.
  • Client Support: Provide emotional support and guidance to clients, addressing their concerns and needs in a sensitive and compassionate manner.
  • Discharge Planning: Coordinate discharge planning, ensuring a smooth transition for clients back into their daily lives.
Requirements
  • Experience: Proven experience working in a clinical environment, preferably in a similar role.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and external stakeholders.
  • Compassion: A compassionate and empathetic approach to client care, with a strong commitment to delivering exceptional support and service.
