About the Role

We are seeking a highly organized and communicative individual to join our Community Gateway Service as a Community Clerical Officer. This role will be based at Crickets Lane Clinic, Ashton, and involves working a shift pattern, on a rota basis, to provide administrative support to our teams.

Main Responsibilities
  • Provide first point of contact for service users making initial contact to the Community Gateway regarding health and/or social care matters.
  • Manage referrals to the District Nursing Neighbourhood, the Integrated Urgent Care Team, and the Adult Social Care Neighbourhood teams.
  • Work closely with our teams to ensure seamless communication and coordination of services.
