Position Overview

We are seeking a dedicated Family Medicine Physician to join our team. The successful candidate will:

Provide comprehensive General Medical Services and Enhanced Services while collaborating with the GP Collaborative, adhering to practice contracts and guidelines.

Be available to perform a range of responsibilities, including in-person consultations, telephonic consultations, home visits, managing repeat prescriptions, and addressing queries and correspondence efficiently.

Participate in on-call duties for urgent clinical issues, ensuring inclusion in the doctors' on-call rota.

Make independent, professional decisions regarding patient care, whether self-referred or referred by other healthcare professionals.

Manage all aspects of patient care within the practice, including initiating diagnostic tests, reviewing results, and making appropriate referrals to secondary care or other services.

Collaborate with patients to develop individualized care and treatment plans, aligning with current practice disease management protocols and broader guidelines such as those from NICE.

Contribute to the practice's achievement of quality improvement frameworks and key performance indicators.

Main Responsibilities

The role will encompass a blend of routine and on-call sessions, with equitable distribution of on-call responsibilities among all physicians.

Our practice offers a combination of appointment types, including both same-day face-to-face and telephone consultations, with an average of 30 patient interactions daily.

Utilize Engage Consult as our workflow management system, supported by a dedicated care navigation team to ensure efficient task allocation throughout the day.

We are transitioning to EMIS Web, enhancing our capabilities, including electronic prescribing.

As a progressive practice, we prioritize efficiency and clinician well-being, fostering a sustainable work environment for all staff.

As a training facility, we currently host registrars, providing opportunities for all physicians to engage in their development through direct management and tutorial workshops, with designated time for these activities.

About Our Practice

Our GP Practice in Wales is a welcoming and innovative facility, part of the Upper Valleys cluster. We are looking to expand our close-knit team, which includes GP partners, salaried physicians, an Advanced Nurse Practitioner, practice nurses, a pharmacist-led pharmacy team, healthcare support workers, and a dedicated administrative staff.

We aim to enhance the quality of care provided to our diverse patient population.

Key Responsibilities

The following duties are expected within this role:

Clinical Duties:

The post-holder will:

Be available for various tasks, including consultations, home visits, managing prescriptions, and addressing patient inquiries promptly.

Participate in on-call duties as required.

Make independent clinical decisions regarding patient presentations.

Conduct screenings for disease risk factors and early signs of illness.

Oversee comprehensive patient management, including investigations and referrals.

Develop care plans in collaboration with patients, adhering to practice protocols and NICE guidelines.

Ensure timely follow-up for patients.

Maintain accurate and detailed electronic consultation records.

Collect data for quality audits.

Utilize computer-generated prescriptions to minimize handwritten notes.

Work collaboratively with the primary healthcare team to deliver high-quality care.

Support the application of standards set by relevant professional bodies.

Contribute to achieving quality improvement frameworks and performance targets.

Organizational Responsibilities:

Comply with all relevant practice policies and guidelines, including those related to confidentiality and data protection.

Commit to ongoing professional development and quality improvement.

Participate in training and educational activities as required.

Assist in the training of staff and the education of medical trainees.

Other duties as agreed with practice partners.
