We are delighted to offer this innovative post at Morriston Hospital Emergency Department, with the opportunity to work on the Emergency Medical Retrieval and Transfer Service (EMRTS) Cymru and Cardiff Physician Response Unit (PRU).

These posts are aimed at strengthening the links between Emergency Medicine and PHEM, both within the arena of critical care and community care. It is intended that these posts in the South Wales region will provide an excellent learning opportunity and will enhance individuals CV’s which will attract high calibre candidates.

The post will commence 7th August 2024 - 5th August 2025

Main duties of the job

The post offers an excellent opportunity to build portfolios and experience in PHEM. Completion of the post is expected to deliver competitiveness for recruitment into PHEM training programmes.

The post would also be suitable for those wishing to take time out of programme (OOPE), although this must be discussed in detail with relevant current training programme directors.

This is an exciting time for PHEM in Wales. The Emergency Medical Retrieval Service went live in 2015, offering an advanced range of prehospital interventions including RSI and blood transfusion, as well the provision of time critical transfers between healthcare facilities and neonatal support to home deliveries and mid-wifery led units.

This post has an average of just over 40 hours per week of which 80% is clinical and 20% SPA. Each component is further divided to 80% EM 20% PHEM.

Please not that this post is not recognised for training but the participate will be able to attend teaching and undertake audit and QI projects as the service needs permit

Working for our organisation

Swansea Bay University Health Board has responsibility for the health of around 390,000 people in the Neath Port Talbot and Swansea areas, with a budget of around £1 billion and employing 12,500 people. We are a University Health Board working in partnership with Swansea University, Swansea School of Medicine, the School of Health Science and the Institute of Life Science. The HB has three major hospitals providing a range of services: Morriston and Singleton hospitals in Swansea and Neath Port Talbot hospital in Baglan, Port Talbot.

Detailed job description and main responsibilities

Our emergency department fosters a strong multidisciplinary team ethos. We have a focus on achieving sustainable working arrangements and will work with you to develop flexible working patterns. We also have a focus on wellbeing, recognising the pressures of working in a busy ED, we have access to a number of support networks. We aim to help you achieve your full potential.

 The department has close links with a number of specialties with the department having consultants accredited in intensive care medicine, paediatric emergency medicine and pre-hospital care and the Welsh Centre for Emergency Medicine Research based in the department. In addition we have consultants with further qualifications in quality improvement, education and change management, this allows us to support individuals wishing to develop skills or a career in a wide range interests.

Emergency Medicine

The post holder will participate in a full shift 24/7 rota of more experienced doctors who provide support to the junior grade at Morriston ED.

You will be clinically responsible for the diagnosis, supervision and management of patients attending the ED.

Provide initial treatment and assessment of adult and paediatric patients presenting with illness or injury and where necessary arranging for patients to be reviewed or admitted by medical staff from other specialties.

On exceptional occasions, be responsible for such irregular commitments outside normal hours as are essential for continuity of patient care.

The appointment is subject to the national medical and dental terms and conditions of service.

The post will be remunerated on the NHS MN37 pay scale, at a point, which matches NHS training grade pay and progression for the individual concerned, banded at 50%.

Prehospital Medicine

Senior Clinical Fellows will undertake fixed sessions with the EMRTS at Wales Air Ambulance bases in Llanelli and Welshpool, by road and air response. Clinical Fellows will always work with a consultant and/or a Critical Care Practitioner.

This experience will be ideal for those with limited previous exposure to aeromedical work. Full Medical Passenger training will be given, as well as training on helicopter etiquette, use of radios, and response procedures. Personal Protective Equipment including a fitted flight suit and boots will be provided. When responding by air, fellows will work alongside EMRTS, one of the World’s most advanced pre-hospital and retrieval services

There will be an expectation that fellows will contribute to the development of EMRTS, by writing/reviewing Standard Operating Procedures, undertaking audit, and contributing to monthly Clinical Governance days and training. All of this will be supervised by PHEM Consultants from the EMRTS.

Due to the physical nature of prehospital care, prospective fellows are required to pass the fitness assessment undertaken by all members of the Emergency Medical Retrieval Service. The assessment consists of a timed one mile run with a 20kg rucksack, followed immediately by a prolonged CPR exercise and patient lift. Critical Care Practitioners of the Emergency Medical Retrieval Service will moderate the assessment.

MB BS or recognised equivalent GMC Reg with license to practice Evidence of working towards Primary MRCEM, Primary FRCA or MRCP.

MRCEM, MRCP, Primary FRCA Provider status in ALS/ATLS/EPLS Academic Excellence (prizes, merits, distinctions etc.)

Experience

Evidence of ability to work at a Specialist Registrar/Specialty Doctor level in Emergency Medicine.

4 years post-graduation experience in specialties relevant to Emergency Medicine Evidence of training in leadership of Trauma and Cardiac Arrest Teams Experience in Pre-hospital Medicine/Paediatrics/Acute Medicine/Trauma & Orthopaedics/Anaesthesia/Intensive Care
