We are seeking a highly skilled and experienced Patient Care Specialist to join our team at University Hospitals Bristol and Weston NHS Foundation Trust. As a key member of our nursing team, you will be responsible for providing high-quality patient care and leading a team of nursing professionals.
Key Responsibilities:
- Provide expert-level patient care using a flexible patient-centered care approach.
- Lead a team of nursing professionals, providing advice, information, and support.
- Assume responsibility for undertaking tasks delegated by the sister/charge nurse, including mentoring roles, auditing, and clinical governance.
- Act as a clinical expert, providing specialist knowledge and advice to nurses and junior medical staff.
- Work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on.
- Be proactive in supporting the nursing team and acting as a positive role model, promoting best clinical and professional practice.
- Demonstrate excellent communication and leadership skills, actively involved in recruitment and staff development reviews.
- Assess, plan, implement, and evaluate evidence-based, individualized patient care.
- Manage patient care according to agreed policies and protocols, taking appropriate action and seeking advice from other members of the multidisciplinary team as necessary.
