About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Kingswood Health Centre. As a Care Coordinator, you will play a vital role in supporting patients with enhanced needs to access the services and support they require to improve their health and wellbeing.

Key Responsibilities

  • Liaise with practice teams to ensure proactive call and recall systems for patients with Learning Disabilities, booking and completing annual health checks, and providing information to help them get the most from their review.
  • Proactively engage with patients, carers, and practices to ensure maximum uptake of reviews, and follow up any patients who fail to attend appointments.
  • Act as a link between Support Workers, the GP, and other healthcare colleagues, supporting onward referrals, care planning, and administrative needs as appropriate.
  • Support patients with Serious Mental Illness, booking and completing mental health reviews, and ensuring that contacts with the patient are recorded appropriately.
  • Administer weight management services, identifying suitable patients, making contact, and gathering baseline information as required.

Requirements

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Knowledge of healthcare systems and services.
