As Care Manager, you will be responsible for delivering exceptional care, ensuring clients remain in their own homes and maintain their independence for as long as possible. Regular travel to clients in your area is required, hence a driver with their own car is essential.

Active Care Group has extensive experience providing high-quality, complex care for people in their own homes, taking into account their clinical, personal, social, and emotional needs. Our care packages are tailored to meet each individual's needs, preferences, and lifestyle.

Key Responsibilities:

  1. Own and manage care packages in your area
  2. Collaborate with the Business Development Manager to review suitable referrals
  3. Produce comprehensive assessment reports for care packages
  4. Buildexcellent working relationships with clients, families, and external stakeholders
  5. Ensure high-quality, client-focused care is delivered at all times
  6. Support recruitment and training for Support Workers for care packages in your area
  7. Ensure accurate and timely completion of records

Requirements:

  1. NVQ 4 or above in Health and Social Care
  2. Full clean driving licence with access to own car
  3. Passion for delivering high-quality, client-focused care
  4. Experience in care at Team Leader level or above
  5. Previous management experience is advantageous
  6. Excellent verbal and written communication skills
  7. A positive attitude and commitment to continuous improvement and quality
