Job Description

We are seeking a highly skilled Community Care Coordinator to join our team in Bristol, North Somerset, or South Gloucestershire.

In this role, you will be responsible for coordinating the delivery of community respiratory services, including pulmonary rehabilitation programmes, Home Oxygen Assessment Service, and the NHS@home respiratory pathway.

You will also work closely with colleagues to develop and implement audit and evaluation methods to improve the quality and efficiency of care for relevant patient groups.

Key Responsibilities
  1. Coordinate the delivery of community respiratory services, including pulmonary rehabilitation programmes.
  2. Provide specialist advice and support to healthcare professionals across BNSSG.
  3. Develop and implement audit and evaluation methods to improve care quality and efficiency.

The successful candidate will have a strong background in community care and the interface with secondary care, as well as knowledge of audit and evaluation methods.

Estimated Salary: £32,000 - £42,000 per annum depending on experience and qualifications.
