About the Role:

We are seeking dedicated social workers who possess the empathy and determination to create meaningful change. Your role will involve innovative approaches to assist individuals in achieving their goals and maintaining independence in their chosen living environments.

We require a permanent, part-time Social Worker to serve across Lancashire within our Adult Services, focusing on Older Adults and Physical Disability Community Teams.

Your Responsibilities:

In this dynamic role, you will conduct strength-based, outcome-focused assessments and develop support plans, ensuring that residents receive appropriate assistance. You will be instrumental in connecting individuals with local services, fostering independence, and promoting health and well-being.

A significant aspect of your position will involve collaboration with various agencies, professionals, and informal caregivers to deliver a comprehensive service.

Applicants must be registered with Social Work England (SWE) or have applied for registration. This position is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975, and successful candidates will be required to apply for a 'disclosure' through the Disclosure and Barring Service (DBS).

Additionally, you will need to provide a vehicle for use in connection with your duties and must be insured for business use. However, we may consider applicants who cannot drive due to a disability.
