Care Coordinator

Care Outlook LTD is seeking a highly skilled and dedicated Care Coordinator to join our team in Greenwich. As a Care Coordinator, you will play a vital role in maintaining and developing our domiciliary service, ensuring the highest quality of care for our service users.
Key Responsibilities:- Roster and template care calls using an electronic system to ensure efficient and effective care delivery.
- Direct management of the care team, providing supervision, appraisals, and training to support staff in delivering exceptional care.
- Create, manage, and monitor customer care plans, ensuring staff rotas meet service users' needs and aspirations.
- Ensure care assessments, care, and risk planning meet best practice and customers' aspirations.
- Work closely with the Recruitment Team to ensure sufficient current and future staffing levels.
- Provide care services to service users in an emergency, arrange emergency cover when required, and cover all/any calls reasonably where required.
- Ensure telephones are answered promptly, and people are spoken to in a polite and respectful manner.
- Work alongside the Branch Manager to ensure all quality standards, complaints, and comments are dealt with promptly.
- Participate in the paid on-call rota.
- Implement measures to comply with CQC regulations and work towards achieving the best care standards possible.
- Competitive salary of £23,500 - £25,000.

- Knowledge of CQC standards and CQC compliance.
- Experience with rostering care calls on an electronic system.
- Experience in care coordinating or 2 years' working within health and social care.
- Full driving license and access to a vehicle (preferred).
- NVQ Level 3 Health and Social Care (preferred).
