About the Role

We are seeking a dedicated and compassionate Healthcare Support Worker to join our team at Sirona care & health CIC. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals in need of rehabilitation and support.

Key Responsibilities
  • Assist registered nurses and healthcare professionals in the delivery of individualized holistic care, promoting dignity and respect for all patients.
  • Contribute to the prevention of harm by participating in risk assessments, such as nutrition, pressure area care, continence, and falls.
  • Support patients with their care and nutritional needs, ensuring accurate and safe recording of physiological observations.
  • Work collaboratively with the multidisciplinary team to promote a culture of ambulatory care, encouraging patients to be as independent as possible.
  • Assist in the provision of prescribed programs of care, including rehabilitation and therapy goals, under supervision and guidance.
  • Support patient-centered rehabilitation to facilitate safe and timely discharges, reflecting individual choice and maximizing independence.
