Position Overview

The Sussex Community NHS Foundation Trust is seeking a dedicated Clinical Support Practitioner to join our Community Respiratory Service (CRS). This role offers a 30-hour work week and is classified as a band 4 position, allowing you to cultivate a diverse set of clinical competencies.

The CRS operates as a vibrant and supportive multidisciplinary team, focused on empowering patients to enhance their self-management skills and minimize avoidable hospital visits. Your responsibilities will be multifaceted, primarily centered around the Pulmonary Rehabilitation initiative. Duties may encompass patient assessment, evaluating suitability and commitment, assisting with exercise programs (both at home and in group settings), monitoring vital signs and blood gas levels, instructing patients and staff in essential skills, participating in multidisciplinary team discussions, and contributing to quality audits.

Key Responsibilities

The Clinical Support Practitioner will deliver a combination of general and specialized care to a designated group of patients, demonstrating proficiency and competence that exceeds that of a Clinical Support Worker or Rehabilitation Assistant. You will collaborate closely with registered nurses and allied health professionals.

Your role will be pivotal in ensuring the efficient operation of the Pulmonary Rehabilitation Service, providing guidance, education, and support to patients while ensuring a seamless patient journey from referral to discharge.

Applicants should hold a Foundation Degree in Health and Social Care or a comparable qualification. You will also be expected to achieve proficiency in your practice area through specific competency assessments.

Supervision and mentorship of junior clinical support staff will be integral to your role.

Job Responsibilities

Utilize professional judgment to assess individual health conditions, recognizing changes and seeking assistance from registered nurses or allied health professionals as necessary.

Deliver high-quality clinical and therapeutic care to a patient caseload, adhering to established guidelines and within the scope of practice.

Perform specific interventions as per competency achievements to enhance care delivery, including but not limited to: ambulatory oxygen assessments, capillary blood gas analysis, and nutritional risk evaluations.

Demonstrate competence in advancing exercise programs in collaboration with specialized physiotherapists.

Maintain accurate and legible documentation in compliance with trust policies.

Continuously develop clinical skills relevant to the role through training and competency achievement.

Conduct specific and holistic assessments as delegated by registered nurses or therapists, adhering to established guidelines.

Take responsibility for personal development, ensuring compliance with mandatory training and relevant professional development, while maintaining an up-to-date CPD portfolio.

Participate in organized training sessions and complete all required competencies within designated timeframes.

Engage in regular reviews with your supervisor and participate in annual appraisals following the Knowledge Skills and Framework review process.

Act as a professional role model and undertake link roles as required.

Qualifications

Essential

  • GCSE A-C in Mathematics and English or equivalent.
  • Foundation Degree in Health and Social Care or a similar qualification in health or exercise.
  • Demonstrated proficiency in literacy and numeracy.

Desirable

  • Qualifications in exercise science.

Experience

Essential

  • A minimum of three years' experience in a clinical support role.
  • Experience in communicating effectively with diverse populations.
  • Skills in patient assessment.
