Children's and Young People's Practitioner

Spencer Clarke Group is seeking a highly skilled Short Breaks Practitioner to lead the coordination of short breaks and social care packages for children and young people with disabilities.
Key Responsibilities- Act as the lead professional and care coordinator for a caseload of disabled children with complex needs.
- Collaborate with internal teams and external agencies to deliver holistic, coordinated support for children and families.
- Develop and implement effective short breaks and social care packages to meet the needs of children and young people.
- Provide expert guidance and support to families and carers to ensure the best possible outcomes for children and young people.
- Stay up-to-date with relevant legislation, including the Children Act 1989, Children and Families Act 2014, and SEND framework.
- Experience working with children with disabilities and their families.
- Strong knowledge of relevant legislation and policies.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with internal teams and external agencies.
- Contract type: 3-6 months minimum, with a high likelihood of extension.
- Hours: Monday to Friday, 37 hours per week.
