Band 6 Community Practitioner

University Hospitals of Leicester NHS Trust is seeking a highly skilled and experienced Band 6 Community Practitioner to join our team in the Emergency Department High Frequency User Program.
Job Summary:The successful candidate will work collaboratively across Clinical Management Groups (CMGs) and with community partners to identify and proactively work with individuals who are High Frequency Users of the Emergency Department. The practitioner will promote and advocate for the care provision and the smooth transition of patients in the acute care areas, ensuring that patients are cared for in the appropriate environment.
Key Responsibilities:- Work autonomously as part of the University Hospitals Of Leicester High frequency Users program and the Leicester Royal Infirmary Emergency Floor, building and maintaining relationships and networks with Partners both internal to UHL and external within the Community.
- Identify and respond to unmet needs in those individuals who present as High Frequency Users to the Emergency Department, advocating on behalf of patients to promote safe and timely discharge from the Emergency Department and to avoid admission into hospital.
- Ensure that robust systems are in place for effective monitoring of performance against national and local indicators and any other reporting requirements, being familiar with all local and NIHCE guidance relevant to the speciality.
- Develop and maintain an accurate and effective record keeping system, collecting statistical data and writing reports on a regular basis, and presenting to appropriate forums within and external to Trust.
The ideal candidate will have:
- Current NMC or HCPC registration with no restrictions to practice.
- Qualification in teaching/mentoring.
- Significant experience at band 5 or above in specialist area, or relevant area with transferable skills.
- Experience in discharge planning.
- Ability to work autonomously and as part of a team, with excellent communication both written and verbal.
