About This Role

We are seeking a highly skilled and passionate Registered Nurse to join our Community Diabetes Specialist Service as a vital member of the team. You will work alongside Specialist Nurses, Care Technicians, Educators, and Administration staff to deliver exceptional diabetes care within the community and primary care settings.

Your Expertise Matters

As a Diabetes Specialist Nurse, you will possess in-depth clinical knowledge and expertise. You will be responsible for providing comprehensive diabetes management to patients with Type 1 and Type 2 Diabetes. This includes insulin initiation and titration, GLP1 therapy management, insulin pump management (South Division only), carbohydrate counting, and structured education for individuals with Type 1 Diabetes.

Key Responsibilities

  • Provide expert diabetes care and support to patients, assessing their complex needs and developing personalized care plans.
  • Deliver exceptional clinical care, including insulin initiation and titration, GLP1 therapy management, and carbohydrate counting.
  • Educate patients on diabetes management strategies and empower them to take an active role in their health.
  • Collaborate with multidisciplinary teams to ensure seamless patient care.
  • Contribute to service improvements and initiatives within the team.
