The successful candidate will operate autonomously for the majority of their role, receiving guidance from the Clinical Director and General Practitioners within the Meadowgreen Health Centre network. This position involves collaboration with GP Partners and Practice Managers across the associated practices.

Key Responsibilities

This role is essential for enhancing the quality of patient care and operational effectiveness, necessitating a strong commitment and enthusiasm to provide outstanding service within the healthcare setting.

The individual will be encouraged to advance their professional development to achieve non-medical prescriber status if they do not already possess this qualification.

Core Duties

- Formulate and oversee a comprehensive medication management strategy and deliver patient services in accordance with practice and network policies, as well as local and national guidelines.

- Lead and manage the medication management team, including the provision of training, to optimize cost-effective prescribing and enhance the quality of patient care.

- Collaborate within a multidisciplinary team to offer expertise in clinical medication management, conduct structured medication reviews, manage chronic conditions, oversee medication during transitions of care, and implement systems for safer prescribing.

- Conduct medication reviews for patients with multiple medications, particularly focusing on older adults, individuals in care facilities, and those with various co-existing health issues.

- Provide leadership in quality improvement initiatives and clinical audits, as well as manage certain aspects of the Quality and Outcomes Framework (QOF).

- Deliver medication review services to patients through clinics, home visits, and in residential and nursing facilities, ensuring the development of pharmaceutical care plans that promote cost-effective prescribing and improve patient care quality.

- Engage with patient groups or other relevant events to provide guidance on the appropriate use of medications.

- Create patient information materials and conduct medicine awareness initiatives throughout the year.

- Assist partners in the effective monitoring and management of their prescribing budgets.

- Prepare evidence-based resources and information to support the medication management team and other healthcare professionals in implementing rational and cost-effective prescribing practices.

- Contribute to the planning, development, and support of new operational processes within the practice to enhance the quality of prescribing.

- Assess patients with single or multiple health conditions requiring medication optimization, including those related to respiratory, cardiovascular, and diabetes care.

- Review the ongoing necessity for each medication, monitor requirements, and support patients in their medication management to ensure optimal use of their prescriptions.

- Make informed recommendations to Senior Pharmacists or General Practitioners regarding medication improvements.

- Conduct clinical medication reviews with patients and provide recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.

- Collaborate with care home staff to enhance the safety of medication ordering and administration processes.

Candidate Profile

Preferred Experience

  • Background in Primary Care or GP Practice.
  • Familiarity with GP clinical systems within Primary Care.
  • Experience with prescription processes and prescribing systems.
  • Involvement in implementing clinical audits.

Qualifications

Essential

  • Master’s degree in Pharmacy (MPharm).
  • Registered Pharmacist with the General Pharmaceutical Council.
  • Ideally, 2-3 years of post-registration experience.

Preferred Qualifications

  • Specialized knowledge gained through postgraduate diploma or equivalent training/experience.
  • Qualification in the primary care pharmacy education pathway, including independent prescribing or working towards this qualification.
