Are you a qualified expert in Physiotherapy, Social Work, Occupational Therapy, or Nursing? If so, Bush & Co is looking for passionate individuals to fill the role of Clinical Case Manager.
Position Overview:
- Oversee a portfolio of clients throughout their recovery process
- Conduct evaluations and establish objectives for clients in their residences
- Champion the necessary support and services to enhance clients' health and community integration
- Manage all documentation and administrative responsibilities related to client cases
Qualifications for Success:
- At least 5 years of relevant clinical experience
- Current registration with a recognized clinical body such as HCPC, NMC, or SWE
- Experience with rehabilitation programs is essential
- Exceptional skills in building relationships
- Ability to travel throughout the UK as required
