About the Role

We are seeking a highly skilled and experienced Clinical Associate Case Manager to join our team at Bush & Co. As a Clinical Associate Case Manager, you will play a critical role in supporting individuals who have experienced life-changing injuries and illnesses.

Key Responsibilities
  • Manage a caseload of clients through their rehabilitation journey
  • Lead on assessments in the home and rehabilitation planning
  • Signpost and advocate for clients' needs, ensuring they receive the necessary support to live a fulfilled life
  • Manage all administrative tasks related to cases, including reporting and communication with third parties
Requirements
  • At least 5 years of clinical experience
  • Valid clinical registration with a relevant professional body
  • Previous experience in establishing or implementing rehabilitation packages
  • Strong relationship-building skills and ability to work in a proactive, solution-focused manner
  • Driver with access to a vehicle and willingness to travel extensively across the UK, Scotland, and Wales
