We are seeking a highly skilled and motivated Band 5 Occupational Therapist to join our dynamic Rapid Response and Intermediate Care Team. As a key member of our team, you will work closely with a multidisciplinary team of healthcare professionals to provide high-quality patient care in the community and in-patient rehabilitation settings.
Key Responsibilities
- Undertake holistic initial assessments with patients to identify their needs and goals.
- Develop and implement individualized treatment plans to promote patient independence and well-being.
- Liaise with patients, families, and carers to ensure effective communication and collaboration.
- Delegate tasks to support worker colleagues to ensure efficient and effective care delivery.
- Attend and participate in clinical meetings, team meetings, and in-service training to stay up-to-date with best practices and guidelines.
