About the Role

We are seeking a skilled and dedicated Band 5 Occupational Therapist to join our Rapid Response and Intermediate Care Team at Sheppey Community Hospital.

As a key member of our team, you will work closely with Occupational Therapists, Physiotherapists, Nursing, and Support Worker colleagues to provide high-quality patient care.

Our service operates 7 days a week, 8am-8pm, and includes weekend and bank holiday working.

Main Responsibilities

  • Undertake 6-month rotations into Intermediate Care, Rapid Response, and In-patient Rehabilitation Unit
  • Complete holistic initial assessments with patients
  • Develop treatment plans in line with patient goals
  • Liaise with family members, relatives, and carers
  • Implement and deliver treatment interventions
  • Complete follow-up review visits with patients
  • Delegate to Support Worker colleagues
  • Attend and participate in clinical meetings, team meetings, and in-service training
