About the Role

Hazelmeade House is a well-established housing project that provides support to individuals recovering from mental health issues. Located in Swindon's old town, it offers easy access to local amenities and the town centre.

As our Senior Support Worker, you will play a key role in delivering person-centred, strengths-based support to our customers. You will lead and inspire a team of Support Workers to do the same, managing a caseload of customers through their journey and supporting them to achieve their goals.

Key Responsibilities:

  • Manage referrals into the service, undertake interviews and assessments, and notify potential customers of the outcomes of these activities.
  • Induct new customers into the service and deliver quality, outcome-focused support sessions in both one-to-one and group settings.
  • Develop and implement individualised support plans that cater to the unique needs and aspirations of our customers.
  • Collaborate with internal stakeholders to ensure seamless service delivery and customer satisfaction.

Requirements:

  • Hold a Level 3 qualification in Health and Social Care (or equivalent).
  • Be an inspiring, motivated, and supportive leader with a proven track record of working with vulnerable young people.
  • Have excellent communication and interpersonal skills, with the ability to build strong relationships with customers, colleagues, and external partners.
