About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our Sefton Acute Visiting Service as a Clinical Lead. This is an exciting opportunity to work in a dynamic and innovative healthcare organization, providing high-quality care to patients in their own homes and care homes.

Key Responsibilities
  • Lead a team of clinicians in providing urgent care and primary care services to patients in Sefton.
  • Develop and implement effective care plans to meet the needs of patients with complex and acute conditions.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Provide clinical leadership and guidance to junior staff members.
  • Contribute to the development of policies and procedures to improve patient care and safety.
Requirements
  • Registered first-level nurse, paramedic, pharmacist, or other Allied Health Professional with an MSc in Advanced Clinical Practice or equivalent.
  • Experience in leading a team of clinicians and managing quality improvement initiatives.
  • Strong clinical leadership and communication skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
