Job Title: Advanced Community Practitioner

We are seeking an experienced Advanced Community Practitioner to join our team in the Sefton Community Respiratory Team (CRT). The successful candidate will be responsible for providing professional leadership to the team, acting as a role model and demonstrating up-to-date knowledge of respiratory health issues.

Main Responsibilities:
  1. Provide expert clinical leadership to the team, ensuring high standards of care delivery.
  2. Promote the mental and physical well-being of Service Users by ensuring health promotion, psychological, rehabilitation, and educational needs are addressed.
  3. Lead and coordinate the work of Care Teams, ensuring that their activities encompass all facets of Service Users care in line with all Trust and local policies and procedures.
  4. Actively seek the views of Service Users and Carers in the development and monitoring of services.

Requirements:
  • 3 years + Respiratory experience
  • Primary care experience
  • Non-Medical Prescriber
  • Clinical examination / diagnostics
  • Masters in advanced Practice or willing to work towards
Experience:
  • High level of communication skills
  • Coaching / clinical supervision experience
