About the Role

We are seeking a skilled Care Responder to join our Home to Decide Team at South Gloucestershire Council. As a Care Responder, you will play a vital role in supporting individuals in their own homes following a stay in hospital, aiming to improve their quality of life and increase independence.

Key Responsibilities

  • Work with residents to support them at home, promoting independence and improving quality of life.
  • Develop and implement care and support plans in collaboration with practitioners.
  • Provide accurate records of visits in line with Council guidelines.
  • Work closely with practitioners to implement care and support plans, referring issues and discussing concerns as necessary.
  • Visit residents in their own homes, providing a response service 7 days a week between 7.00am – 10.00pm.

Requirements

  • Demonstrate sensitivity and empathy when working with individuals with varying needs.
  • Communicate effectively both verbally and in writing.
  • Hold a clean and valid driving licence and own transportation, with mileage expenses paid.
  • Experience in the care sector is desirable, with knowledge of relevant Care Act legislation and regulations.
  • Hold a relevant vocational qualification, such as RCF - LEVEL 2, and/or a CQC Care Certificate.
