Are you a dedicated healthcare professional with a passion for enhancing the lives of elderly patients? We are seeking a Geriatric Care Nurse to become a vital part of our dynamic team at Rural Warwickshire North Primary Care Network.

This role presents an exciting opportunity for a committed Registered Nurse to collaborate closely with Community Matrons, delivering essential services to individuals experiencing moderate to severe frailty and managing long-term health conditions.

Your contributions will be crucial in evaluating patient frailty levels and facilitating reviews to ensure optimal access to necessary care and support.

As an independent practitioner, you will hold delegated responsibilities for a specific patient caseload.

We are looking for a candidate who is flexible, possesses a proactive attitude, and is dedicated to providing exceptional care while thriving both independently and as part of a collaborative team.

This position is community-focused, and candidates must have a valid driving license and reliable access to a vehicle for daily use.

Key Responsibilities

As a Geriatric Care Nurse and Team Leader, your responsibilities will include:

  • Collaborating with other healthcare professionals to deliver comprehensive care to homebound patients and residents in care facilities.
  • Supporting the virtual ward initiative by providing care to patients enrolled in the frailty pathway.
  • Conducting assessments, planning, implementing, and evaluating patient care.
  • Exhibiting advanced knowledge and practical skills in geriatric care.
  • Participating in multidisciplinary team meetings to discuss and provide feedback on patient care.
  • Utilizing strong communication and interpersonal skills to foster collaborative relationships with patients and healthcare providers.
  • Demonstrating enthusiasm and the ability to take initiative in all aspects of care.
  • Engaging in educational activities and staff development, including regular supervision and performance reviews.

We are a Primary Care Network (PCN) comprising six practices that work collaboratively with various local providers, including primary care, community services, social care, and the voluntary sector, to deliver personalized and coordinated health and social care to our community.

Job Responsibilities

This job description outlines the primary duties associated with the role. The successful candidate will be expected to undertake additional responsibilities as required, in line with service developments.

Core Objectives

  • To operate within Integrated Neighbourhood Teams under the guidance of Community Matrons, providing essential services to individuals with moderate to severe frailty and long-term conditions.
  • To assess the health requirements of designated clients and develop, implement, and evaluate care programs tailored to their needs.
  • To contribute to the public health initiatives within the team.

Essential Qualifications and Experience

  • Registered General Nurse (RGN) with a nursing degree or diploma.
  • Demonstrated nursing experience across various healthcare settings.
  • Experience in delivering patient education and promoting self-management of health.
  • Proven ability to work independently and assess, plan, and implement care for frail patients.
  • Current driving license and access to a vehicle during working hours.
