About the Role
We are seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team at Station Street Surgery, with responsibilities spanning two well-established sites. This role offers an exciting opportunity for a nurse who is passionate about patient care and looking for a long-term position with the potential for significant career progression.
Main Responsibilities
- Deliver high-quality nursing care, including chronic disease management, immunizations, wound care, and health promotion.
- Collaborate closely with GPs, fellow nurses, and other healthcare professionals to provide comprehensive patient care.
- Conduct thorough patient assessments, develop and implement care plans, and regularly evaluate patient outcomes.
- Provide support to patients in managing their health conditions, offering guidance on leading healthier lifestyles.
- Maintain accurate and detailed patient records while ensuring confidentiality is strictly upheld.
- Actively participate in team meetings and contribute to the continuous improvement of our services.
