About the Role

We are seeking an experienced Occupational Therapist to join our team at Livewell Southwest, specializing in Housing Adaptations and Disabled Facilities Grants. As an OT, you will work closely with our Housing team to ensure that adaptations are completed in a person-centered way, meeting the individual's long-term needs.

Key Responsibilities
  • Work collaboratively with our Housing team to assess and plan adaptations for individuals, ensuring that their needs are met and their independence is maintained.
  • Develop and implement effective solutions to address environmental barriers, promoting the individual's functional ability and well-being.
  • Collaborate with contractors and Housing Associations to ensure that adaptations are completed to a high standard, meeting the individual's needs and expectations.
  • Conduct regular reviews to ensure that adaptations continue to meet the individual's needs, making adjustments as necessary.
  • Develop and share expertise with the wider Integrated Therapy teams, promoting best practice and knowledge sharing.
Requirements
  • Extensive experience in health or social care, with a focus on Occupational Therapy.
  • Proven ability to work collaboratively with multidisciplinary teams, including Housing and contractors.
  • Strong assessment and planning skills, with the ability to develop and implement effective solutions.
  • Excellent communication and interpersonal skills, with the ability to work with individuals, families, and carers.
  • Knowledge of relevant Housing legislation and processes, including Disabled Facilities Grants.
  • Ability to work autonomously, prioritizing tasks and managing a caseload effectively.
