Pennine Care NHS Foundation Trust is seeking a dedicated and experienced Senior Mental Health Practitioner to join our esteemed Living Well Service in the Tameside & Glossop region.

As a vital member of our multidisciplinary team, you will collaborate closely with colleagues to provide comprehensive support and care to individuals facing mental health challenges within their communities.

Your Responsibilities:
  • Conduct thorough biopsychosocial assessments and Initial Conversations to understand patients' needs and circumstances.
  • Develop and implement effective risk management plans to ensure patient safety and well-being.
  • Deliver brief interventions, including evidence-based coping strategies and problem-solving techniques, to empower patients in managing their mental health.
  • Provide medication support and actively promote access to social and interagency resources to enhance holistic care.
  • Regularly review treatment plans, adapting them as needed to ensure they remain aligned with patients' evolving needs and goals.
Your Qualifications:
  • Possess a recognized qualification in Mental Health Nursing (RMN/RNMH), Social Work, Occupational Therapy, or an equivalent field.
  • Demonstrate significant post-registration experience working with individuals experiencing mental health challenges.
Your Skills and Attributes:
  • Exhibit comprehensive skills in mental health assessment and risk management.
