About the RoleAs a Community Nurse, you will be responsible for assessing, providing, and reviewing nursing care, advice, and information to patients and carers. You will work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using your skills and knowledge as a Registered Nurse.
You will aim to maintain patients in their preferred place of care, enabling them to maximize their independence and optimize their quality of life. You may be expected to hold a designated caseload and deputize in the absence of the Nurse Specialist (Community).
