Position Overview

The Integrated Transfer of Care Hub is seeking to appoint Patient Flow Coordinators to enhance operational efficiency and improve patient experiences related to care transitions.

We are in search of dynamic and compassionate individuals who possess a foundational understanding of the healthcare and social services landscape to facilitate discharges and collaborate within a multidisciplinary team.

This role operates as a continuous service, providing a centralized point of contact across various departments and facilities within the Mid Yorkshire Teaching NHS Trust, ensuring that staff, patients, families, and caregivers are supported in the safe and timely discharge process in conjunction with other healthcare professionals.

Key Responsibilities

  • Ensure the effective execution of discharge protocols throughout the healthcare facilities to guarantee safe and timely patient transitions.
  • Engage actively in ward-based discharge initiatives, including assessments and patient identification for transfers, to maintain a consistent discharge workflow.
  • Collaborate with the discharge team to identify patients for transfer to designated discharge hubs, ensuring bed availability is optimized.
  • Provide comprehensive and precise information regarding patient discharge plans to relevant stakeholders.
  • Work in close partnership with internal and external organizations to facilitate a seamless discharge process, focusing on minimizing delays.
  • Coordinate with clinical site managers to ensure efficient bed utilization.
  • Advocate for best practices in high-quality discharge planning.
  • Utilize problem-solving skills to navigate complex discharge scenarios and develop tailored management strategies.

Communication and Collaboration

  • Maintain confidentiality in accordance with national and organizational standards.
  • Establish and nurture effective communication channels with a diverse range of individuals, including patients, families, and multidisciplinary teams.
  • Convey intricate patient-related information to foster collaborative efforts across health, social care, and housing sectors.
  • Manage and prioritize workload effectively, seeking assistance and escalating issues as necessary.
  • Adapt communication style to address sensitive or distressing situations appropriately.

Documentation and Administration

  • Ensure accurate and timely documentation in line with organizational guidelines.
  • Adhere to legal standards regarding confidentiality and data protection.
  • Oversee the collection and analysis of relevant data to support discharge planning.
  • Execute routine administrative tasks related to discharge processes.

Qualifications and Experience

  • Prior experience in healthcare settings, particularly in roles involving patient interaction.
  • Understanding of patient flow and discharge planning processes.
  • Ability to work independently while achieving designated objectives.
  • Proficient in utilizing IT systems to support discharge activities.
