About the Role

We are seeking a skilled and compassionate Community Healthcare Support Practitioner to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our Early Intervention in Psychosis Service, you will play a vital role in providing high-quality care and support to individuals experiencing First Episode Psychosis.

Key Responsibilities
  1. Assess and develop individualized care plans for a defined caseload of patients, working closely with a designated supervisor.
  2. Act as a Care Coordinator, implementing and adhering to Trust policies on Integrated CPA and Risk Assessment.
  3. Promote the needs of carers, complete carer's assessments, and support local Carer/User Groups.
  4. Administer medication, ensuring compliance with nursing responsibilities and Trust clinical procedures.
  5. Participate in the Team Duty Worker rota and adhere to Trust requirements.
  6. Ensure compliance with approved Trust clinical nursing policies and procedures.

Person Specification
  • Registered Mental Health Nurse, OT, or Social Worker, with valid registration with the appropriate professional body.
  • Relevant experience working with clients with psychosis in Adult and/or CAMHs services.
  • Knowledge of current primary care issues and current policy and guidance relating to Early Intervention in Psychosis services.
