**Job Summary**

Patient safety specialists will be the lead patient safety experts in healthcare organisations, working full time on patient safety. They will be ‘captains of the team’ and provide dynamic, senior leadership, visibility and expert support to the patient safety work in their organisations. They will support the development of a patient safety culture and safety systems and have sufficient seniority to engage directly with their executive team. They will work in networks to share good practice and learn from each other. Patient safety specialists will lead, and may directly support, patient safety improvement activity and ensure that systems thinking, human factors understanding and just culture principles are embedded in all patient safety processes. They will promote patient safety thinking beyond why things go wrong in healthcare, to examining why things routinely go right and how that can be maximised. They will support their organisations’ ‘patient safety partners’ (patient and public representatives specifically involved in patient safety) as identified in the NHS Patient Safety Strategy.

**Duties and Responsibilities**

**Improving quality and outcomes using insight and evidence for improvement**
- Responsibility for / oversight of the implementation of the NHS patient safety strategy within the organisation
- Oversee and support patient safety improvement, ensuring that systems thinking, human factors understanding and just culture principles are embedded in patient safety processes.
- Communicate patient safety issues, including the definition, framing, escalation and presentation of identified issues, risks and proposed improvement approaches at executive/board level.
- Support / lead multi-professional responses to patient safety incidents, tailoring the different approaches required for new or under-recognised issues and wider patient safety challenges needing long-term improvement, ensuring adherence to national policies and enabling timely and good quality reporting.
- Support an approach to patient safety that drives improvement across the patient pathway beyond the organisation’s boundaries, including facilitating multi-agency reviews where required.
- Support / lead the implementation of continuous improvement of quality and impact of incident investigations, currently through the new Patient Safety Incident Response Framework (PSIRF).
- Oversee patient safety improvement programmes that support the NHS patient safety strategy.
- Provide advice to staff at all levels of the organisation on specific / complex / clinically based / professional issues in relation to patient safety, e.g. statutory responsibilities, national standards, compliance enabling a constructive outcome to be achieved.
- Ensure the board has oversight over safety issues via the reporting flow, with critical safety issues being reported direct to the CEO/Registered Manager.
- Produce, provide and receive highly sensitive, highly contentious and complex information where there can be blockers to acceptance or reception. These will need to be presented at relevant committees and management meetings across the Organisation. Attending meetings to enable challenge, discussion and where required escalation to ensure a positive outcome.
- Work with key individuals across the Trust to provide scrutiny and test the robustness of its patient safety frameworks and processes. Identifying gaps in assurance and making recommendations for improvement.

**General**:
To carry out and comply with the prevention and control of infection as per Hospice policies and procedures.

The post holder is responsible for safeguarding the interests of children and adults who they come into contact with during their work. To fulfil these duties, post holders are required to undertake mandatory training to recognise the signs and symptoms of abuse, or neglect, or individuals at risk and to follow Halton Haven policies relating to safeguarding practice including reporting any concerns that they may have.

Take reasonable care for the health and safety of themselves and of other persons who may be affected by their acts or omissions at work.

Co-operate with their employer as far as is necessary to meet the requirements of the legislation.

The role will be required to occasionally work outside of standard office hours, to support internal and external events, with an aim to promote and celebrate volunteering efforts.
