**PURPOSE OF ROLE**:To represent NEWCIS and contribute to the development of the integrated model of community services, providing good Carer support within the Community Support Services locality teams and the Community Resource Teams.

Undertake holistic assessments of carers’ needs where a Carer is providing regular and substantial care to individuals who have health and social care needs.

To work with NEWCIS, statutory services, the voluntary sector, and partners in health and the third sector to identify opportunities to meet Carers identified needs.

Work jointly with Community Support Services practitioners to ensure there is a cohesive approach in supporting Carers and those they care for, support practitioners to develop their own knowledge and expertise in relation to support and services for Carers.

**MAIN DUTIES AND RESPONSIBILITIES**:

- Ensure NEWCIS is represented by supporting development of the integrated model of community services, providing good Carer support within the Community Support Services locality teams and the Community Resource Teams.
- Process referrals to the service which shall be from a variety of sources including the DCC, primary and secondary care including mental health teams, and self-referrals. All referrals will be processed through Denbighshire’s Single Point of Access.
- Carry out a What Matters conversation with carers, identifying carers individual well-being outcomes and provide them with support to achieve their goals.
- Inform carers of the NEWCIS training, counselling, group activities and volunteering opportunities for carers and past carers to enable them to receive peer support whilst developing their skills and confidence.
- Draw up support plans if local authority assistance is required to achieve any of the outcomes identified, and consider innovative ways in which assistance can be provided with the use of support budgets, prevent the deterioration of the caring role
- You will be part of the Duty Rota as required answering calls that are received from carers.
- Where required, attend Complex Case meetings that involve supporting the carer.
- Support practitioners to develop their own knowledge and expertise in relation to support and services for Carers. Where appropriate, work jointly with Community Support Services practitioners to ensure there is a cohesive approach in supporting Carers and those they care for.
- Assist in carer Awareness training sessions for health and social care staff
- Provide effective links with Denbighshire’s Community Navigators and other professionals.
- Ensure all monitoring requirements are adhered to, report and provide regular updates on progress to the NEWCIS Manager in accordance with NEWCIS reporting requirements.
- To maintain confidentiality over personal information relating to individual users of NEWCIS.
- To undertake any other duties as reasonably requested by the Manager of NEWCIS and participate in NEWCIS activities and events throughout the year.
- You will be required to work to the QCF level 4 or above in social care or IAA and maintain the level of qualification necessary for the post.
