The post holder will deliver a service to facilitate the safe and appropriate transition of high-risk service users that are statutorily supervised by HM Prison & Probation Service who meet the Offender Personality Disorder Pathway (OPDP) eligibility criteria and present with particularly complex needs. The post holder will provide transitional support from both custodial establishments and Approved Premises ontothe community and this delivery forms part of theIntensive, Intervention and Risk Management Service, (IIRMS) aspect of the Wales OPD Pathway. This post will form part of a developing Welsh Intensive, Intervention and Risk Management Service, (IIRMS). In the course of their duties will be responsible for: Providing positive intervention during transitional periods that can enhance the success of service users rehabilitation, leading to reduced rates of reoffending and sustained periods of stability in the community.

Improving access and progression through services by supporting transitions between clinical services in secure OPD sites in England and clinical services in Wales, also to improve maintenance of contact between community services of those recalled to custody. Coordinating with key agencies (statutory and Voluntary) within the prison community and the wider geographical community to meet the complex needs of the identified client group. Fostering and developing working and partnership arrangements at relevant groups, meetings, forums providing specialist advice, knowledge on the Transition Offenders. Assist the identify high risk OPDP eligible service users who present with particularly complex needs with their access to appropriate services, resources, treatment and/or support networks within their identified home area.

Provide referrals for support in key areas such as housing, substance misuse interventions, educational opportunities, participation in voluntary and/or permanent work by means of outreaching from the approved premises, and where appropriate access to Mental Health Services where difficulties may be encountered as part of the CTP framework. To identify the exact nature of the needs as identified by service users and staff using the Goals and Plans framework alongside information from probation / psychology reports. Contribute to the development of more effective pathways through attendance at CTP, sentence planning, MAPPA, and MARAC meetings; bringing professionals together; service mapping; awareness raising; facilitating communication and referrals between services; and assisting with 'unpicking' of blockages. Increasing quality of clinical services by supporting the maintenance, generalisation to the community, and the continued development of therapeutic gains; facilitating the communication of information about risk and risk management by enhancing the relational and practical support during the difficult transition from approved premises to the community Identify/facilitate short-term evidence-based interventions working in a trauma informed, recovery focused manner that can be implemented during transition, such as anxiety management, emotional regulation, etc.

Provide strategic and practical support to service users by way of creating a service map and resource / contacts guide as a means to establish links to local services including GPs; housing providers; employment services; primary care; substance misuse services; local support groups etc.Participate in the service's response for consultation, formulation and assessment of clients with complex and high-risk behaviours, who are managed by other agencies within the criminal justice system. Ensure that the physical health needs of specific client group are adequately achieved through collaborative management with Primary Care providers, particularly for those individuals who would normally neglect this aspect of their health needs. Regularly prepare for, attend and participate in multi-professional meetings and other meetings where the service user's progress is discussed as well as collaborative goal setting with service users, other professionals, family members and other support sources whenever possible. Co-ordinate and support connections between existing statutory and third sector service providers substance misuse, support networks, service user groups, Citizens Advice Bureau etc.

in collaboration with OPDP colleagues focus onservice userswho are in custody awaiting transfer to Approved Premises that meet the criteria for the OPDP pathway. Schedule and prioritise sessions working from various Probation Offices within the South Wales area, liaising with Probation Practitioners on a face-to-face basis regarding complex cases. Participate in the development and implementation of robust contingency planning, particularly in relation to crisis intervention. Autonomously manage own caseload ensuring that travel between treatment sites is most effective.
