An exciting position has been created in the Discharge Hub for administrative support to a team of Discharge Coordinators and hospital Social Work teams that oversee complex discharges/transfer of care processes.
- The Internal Flow Hub is formed of the hospital discharge team, the homeless team, Transfer of care navigators, hospital social work teams and Red Cross team. This department sits within the Integrated Care clinical group.

The post holder will be responsible for providing general administrative support to the Discharge Coordinators, carrying out admin triage of Discharge To Assess (D2A) referrals, entering referral details on the adult social work Mosaic systems and regular liaison with out of borough Local Authorities.

The Hub operates a 7 day, 08:00-18:00 service, successful applicants will be expected to work mainly 10 hour shifts with occasional weekend shifts included as part of their rota.

General Duties
- D2A referral admin triage and validating patient records on the D2A tracker.
- Processing Out of Borough referrals for Lambeth and Southwark Adult social work teams.
- Maintain stationary supplies and services in accordance with policies and procedures.
- Sort, distribute and handle incoming mail and in a timely and efficient manner. Ensure that outgoing mail is collected and dispatched. Arrange the receipt and delivery of courier mail and parcels as required.
- Photocopying, collation and distribution of reports, correspondence etc.
- Covering front desk duties as required and answering telephone queries in a professional manner.
- Reporting and monitoring any maintenance / facilities as appropriate.

The Discharge Hub service is composed of a multidisciplinary team of Discharge Coordinators, Social Workers from Lambeth and Southwark, Occupational Therapists, Intermediate Care representative from Lambeth and the Red Cross team. We are based in a non-clinical area on the ground floor, South Wing, St Thomas' Hospital.

This role will support day-to-day complex discharges across the Trust by providing requisite admin support to the multidisciplinary team and providing front of house reception cover.

**Responsibilities**:
The duties and responsibilities listed below are representative of the Administrator role and its purpose within the service; they are not exhaustive and the post holder may be required to undertake additional or alternative administrative duties and responsibilities commensurate with the level of the post, to support the smooth running of the service.

Main duties will include;
- Provide high quality administrative support to the department. This may include, D2A referral admin triage, liaising with Social Work teams to transmit D2A referrals, bookings meeting rooms and hospitality, organising audio-visual equipment, ordering department supplies such as stationery plus any other ad hoc duties.
- Typing and formatting of letters, reports, policies, minutes, presentations, statistics and general correspondence/ documents, as required, using Microsoft Office and other software packages.
- Provide support for meetings, including the preparation and distribution of agendas and taking, transcription and distribution of minutes/ action notes. Follow up of action points arising from meetings.
- Provide administrative support to projects being managed within the service as directed.
- Provide administrative support within the service, including cross cover for administrative team members as required, including deputising for the Team Lead for short periods of time.
- Providing support and leadership as well as supervision to the Band 2 administrators.
- Provide advice and support to new or less experienced employees on own administrative duties.
- Maintain departmental databases in a timely and accurate manner. Assist with the collation of data as required.
- Provide excellent customer care to ensure timely, relevant advice and assistance to all GSTT service users.
