To work within a multi-disciplinary team to facilitate a range of recovery and abstinence
focused interventions for adults with problems relating to co-occurring substance
misuse issues (mental health, housing, criminal justice etc).
The post holder will provide harm reduction and health and well-being interventions
for Stockport residents presenting with a variety of substance misuse issues including
alcohol, opiates, stimulants, Steroids, over the counter, prescription and emerging
drugs. The post holder will offer prescription and care management interventions for
service users working, wherever possible, towards recovery and abstinence using an
asset-based framework.
The post holder will provide clinical interventions supporting sustained recovery within
an ambitious, inspirational and outcome focused culture promoting hope,
empowerment and choice.
- To contribute to the provision and facilitation of a range of interventions to a case load of alcohol and drug clients identified by both substance misuse and specialist service providers. Focusing on managing individual’s recovery capital and increasing their chances of achieving and sustaining abstinence. This will include: psychosocial interventions, provision of substitute medication where appropriate, support to access mutual aid and peer support networks, support to develop positive community relationships and support to access other services both substance misuse specific (e.g. rehab and community programmes) and mainstream (e.g. housing, employment, education and training.)

- Provide advice, education and support to service users, families, carers and

wider communities’ on public health issues relating to substance use and
safety, prevention and health promotion.
- To provide a range of services within a harm reduction model, including

reducing alcohol related harm, safer injecting, sexual health and general
health advice.
- To provide general health assessment, monitoring and interventions (e.g.

blood pressure monitoring, lung function testing) whilst enabling the service
user to access mainstream community health services.
- Provide specialist clinical information, support and advice to other health and

social care professionals and internal and external agencies.
- Support local public health campaigns and engage in public facing substance

misuse and health messages.
- Engage service users in therapeutic alliances and facilitate up take of wider

treatment and recovery services.
- Enable Service users to access Blood Borne Virus screening and

vaccinations for hepatitis A and B and where appropriate Flu vaccination and,
where indicated, facilitate access to these services for the User's family
members.
- Facilitate group work programmes and support the visible recovery

community.
- Provide verbal and written information to ensure service users can make

informed choices about their recovery journey
- Work with service users within local community amenities in order to

maximise individual health and wellbeing potential whilst promoting social
networks and active citizenship.
- Actively engage with carers and significant others both in advising their own

needs and where appropriate as part of the service users care
- Actively engage with the wider community as part of the services undertaking

in order to forge positive recovery networks and develop community capacity
enabling service users to build positive social networks.
- Actively engage with other agencies both across the borough and wider,

liaising on behalf of service users
- Work with the borough’s Children and Family Service in relation to Boroughs

initiatives.
- Provide reports, where required, in relation to Child and Adult safeguarding

proceedings and attend Child In Need and other safeguarding meetings.
