Recovery Co-ordinator

Provide advice, education and support to service users, families, carers andwider communities on public health issues relating to substance use andsafety, prevention and health promotion. To provide a range of services within a harm reduction model, includingreducing alcohol related harm, safer injecting, sexual health and generalhealth advice. To provide general health assessment, monitoring and interventions (e.g.blood pressure monitoring, lung function testing) whilst enabling the serviceuser to access mainstream community health services. Provide specialist clinical information, support and advice to other health andsocial care professionals and internal and external agencies.
Support local public health campaigns and engage in public facing substancemisuse and health messages. Engage service users in therapeutic alliances and facilitate up take of widertreatment and recovery services. Enable Service users to access Blood Borne Virus screening andvaccinations for hepatitis A and B and where appropriate Flu vaccination and,where indicated, facilitate access to these services for the User's familymembers. Facilitate group work programmes and support the visible recoverycommunity.
Provide verbal and written information to ensure service users can makeinformed choices about their recovery journey Work with service users within local community amenities in order tomaximise individual health and wellbeing potential whilst promoting socialnetworks and active citizenship. Actively engage with carers and significant others both in advising their ownneeds and where appropriate as part of the service users care Actively engage with the wider community as part of the services undertakingin order to forge positive recovery networks and develop community capacityenabling service users to build positive social networks. Actively engage with other agencies both across the borough and wider,liaising on behalf of service users Work with the boroughs Children and Family Service in relation to Boroughsinitiatives. Provide reports, where required, in relation to Child and Adult safeguardingproceedings and attend Child In Need and other safeguarding meetings.
Maintain knowledge of local and national criminal justice agenda and otheremerging priorities and incorporate this into practice within the Serviceframework including delivering interventions to support this agenda whereindicated. Ensure effective data collection and information provision including the use ofcomputer based systems Make and maintain clear and concise clinical records compliant with Trust andlocal contractual expectations Liaise with multi-disciplinary teams seeking support and advice as necessary Assist in the development of services where needed Participate in training and other opportunities for the professionaldevelopment of self and others, including the supervision of more junior staffor students Work at all times to promote equality, diversity, and individual rights Support senior workers to ensure that the service supports local and nationaltargets in relation to waiting times, access effective engagement, planneddischarge and TOPs complianceCommunication The post holder will be expected to undertake complex and sensitive drug andalcohol specific conversations both with clients directly and on their behalfwith other agencies. The content of these conversations will include significanttechnical expertise and may on occasions have emotionally distressingelements The post holder will be expected to have and be able to communicate drugand alcohol specific knowledge and expertise The post holder will be expected to undertake these conversations on behalfof more junior staff when the level of expertise lies outside their experience Participate in local substance misuse, health promotion and communityengagement awareness and education campaignsStaffing / Managerial Active contribution to the project management of initiatives arising fromservice development priorities Supervise and support more junior members of staff Participate in and provide regular and appropriate supervision sessionsthat should encompass professional, clinical, performance and serviceissues. Liaise and work with service user representatives and ensure serviceusers are involved in service developments Provide data and reports to demonstrate outcome measures as required Keep abreast of current research, literature and other developmentsrelevant to work with this client group and to your own professional spherePolicy To ensure that the Trust safeguarding policy is implemented To ensure all Pennine Care NHS Foundation Trust policies areunderstood and implemented within the working environment Develop and review protocols, Standard Operating Procedures andGuidance along with Service Users to ensure quality standards are met.
